August 2025, Updated Announcement

Current patients. I am confirming email addresses to send NEW invites to NEW records system and their NEW patient portal. All documents must be renewed and electronically signed.

Open new patients on a case by case basis.

You must have a PCP. I treat age 6 years and up.

Location changed to Seaside, OR August 2025.

Scheduling will be done during office hours, until self scheduling is started, coming December 2025.

Must cancel appointments 48 hours in advance.

I do not provide pet or emotional service animal (ESA) letters. I do not provide Suboxone.
